Facial Fat Grafting
Undo the effects of volume loss to achieve fuller, sculpted definition of the face.
SCOTTSDALE, AZ
Losing fat volume in the face is a common occurrence as we age. The most noticeable volume loss occurs in the midface, along the cheeks, cheekbones, and under eye areas. Over time, this can create a hollowed out or gaunt appearance that makes one look and feel haggard even on their best days.
To address these concerns, Dr. Saba Ghorab offers an “autologous fat transfer” to redistribute the body’s own natural fat, typically from the abdomen or thighs, to volume depleted areas of the face. Unlike synthetic dermal filler injections, Scottsdale facial fat grafting is a natural and long-lasting solution for volume restoration and rejuvenation of the face.
What Can Facial Fat Grafting Help With?
You could be a candidate for a facial fat transfer if you experience:
-
-
- Aging face with loss of volume and structural support and loose, deflated skin
- Hollowing of the face with contour irregularities that cast dark shadows
- Loss of height and fullness of the cheeks and cheekbones
- Sunken grooves and dark circles under the eyes (tear trough deformity)
- Deep creasing of the smile lines with redundant folds of skin
- Irregular depression or concavity due to injury, birth defect, or health issue
-
Fat is Youthful.
Did you know that fat, or adipose tissue, is actually useful to have in your face? It fills out the cheeks and other areas of the face to create a full, youthful appearance.
Time and gravity cause facial fat, especially in the cheeks, to shrink and sink. Loss of fat volume accelerates facial aging, with hollowing of the midface and heavy folds of skin draping over the lower face.
The net effect is unwanted droopy, deflated cheeks with flatter cheekbones, dark hollows under the eyes, heavy smile lines, and jowling. These signs of facial aging can become visible as early as the late 20’s to early 30’s, with unfortunate progression over time.
What is Facial Fat Grafting?
Scottsdale facial fat grafting is a minimally invasive technique that is effective for restoring facial fat volume.
“Autologous” fat, or an individual’s own adipose tissue, is used to re-invigorate the face for youthful, supple volume.
Fat grafting is often combined with eyelid surgery, facelift, or neck lift for more complete rejuvenation of the face, resulting in a lifted, smooth, and supple appearance.

How is Fat Grafting Performed?
During an autologous fat transfer, fat is taken from areas like the belly or thighs using a gentle, manual liposuction technique. The adipose tissue is then carefully processed to collect the “cream of the crop” fat cells only.
Using specialized cannulas, the “liquid gold” is injected into the face to re-create a youthful distribution of fat volume. This has a secondary lifting and tightening effect of the overlying facial skin, thereby improving the appearance of heavy skin folds.
Our minimally invasive approach does not require any major incisions or cuts on the face, thus avoiding unwanted scars. Small incisions at the fat donor site heal quickly with use of stitches that dissolve on their own.
MicroFat Grafting
The success of facial fat grafting is ultimately dependent on the quality and viability of the adipose tissue used to restore facial fat volume. Though many factors go into this, it is prudent for a skilled facial plastic surgeon to use advanced techniques and technology to improve the survival of the grafted fat.
Scottsdale microfat grafting is a technique in which smaller aliquots or sizes of adipose tissue are injected into the face. This requires specialized instrumentation and good injection techniques to minimize cell injury during the procedure. Though a meticulous process, the results are well worth it!
At Saba Plastic Surgery, we routinely use microfat grafting for facial volume restoration to create a fullness that looks and feels natural, soft, and smooth.

What to Expect with Facial Fat Transfer
Facial fat grafting is an elective, outpatient procedure performed in our Surgery Center with licensed personnel. Dr. Ghorab will work 1:1 with you to examine your facial features and give an expert assessment for how to best achieve your aesthetic goals. If a facial fat transfer is recommended, she will review what the procedure entails, the postoperative recovery, and the results you can expect.
Anesthesia
Facial fat grafting is performed using a combination of local and twilight anesthesia. Our minimally invasive anesthesia protocol provides excellent surgical outcomes while patients are comfortably sedated and still breathing on their own. Since there is no need for a breathing tube, the postoperative recovery is much more favorable.
Post-Anesthesia Recovery
After the procedure, patients are monitored in our cozy and relaxing recovery room. Once awake and doing well, patients are taken home by their designated caretaker. A custom aftercare plan is provided and follow-up scheduled.

Recovery After Facial Fat Grafting
Most patients are pleasantly surprised to have a quick and easy recovery after facial fat grafting. We place great emphasis on postoperative follow-up care, ensuring you are closely monitored for optimal healing
Bruising & Bleeding
Minor bruising may occur whereas serious bleeding with hematoma formation is rare.
Swelling & Tenderness
It’s normal for the fat donor areas (i.e. belly or thighs) and the treated areas of the face to feel tender and swollen for the first 1 to 2 weeks after the procedure.
Facial Fullness
The treated areas of the face should look fuller than desired for the first several months to compensate for the inevitable loss of some transferred fat. As such, the treated areas will gradually “settle down” to the desired volume restoration by 4 to 6 months, after which stable longevity of the remaining fat is expected.
Time Off
Following facial fat grafting, we recommend 1 week off to hunker down for R&R.
Pain Control
Any discomfort is typically mild and well-controlled with over-the-counter pain medications only.
Activity
Strenuous activities should be avoided for at least 2 weeks.
Follow-Up
Routine postoperative visits are scheduled, starting 1 week after surgery and for up to one full year.

FAQ: Facial Fat Grafting
An “autologous” fat transfer simply means that an individual’s own fat is removed from one area and injected into another. For facial rejuvenation, the belly or thighs are the most common donor areas for adequate stores of rich adipose tissue that is then injected into the face.
Yes! Any extra fat is that is taken from the donor site but not needed/used for the face can be processed for frozen storage, in case additional volume replacement or touch-up injections are desired in the future.
