Surgical changes made to improve the appearance of a nose will invariably affect its function and vice versa. Though the distinction between a “cosmetic” and “functional” rhinoplasty matters in some instances, both factors must be taken into consideration during a rhinoplasty.

A rhinoplasty is typically combined with chin augmentation and injection with dermal filler and botulinum toxin to improve the overall look of the face.

  Primary vs Secondary Rhinoplasty

 

A primary rhinoplasty means a first time nasal surgery on a “virgin” nose that hasn’t been operated on before.

A secondary or revision rhinoplasty is undertaken in patients with a prior history of nasal procedures. There can be added complexity due to post-surgical scarring and irreversible modifications or absence of nasal tissues such as partial removal of septal cartilage during a previous septoplasty or septum surgery.

Despite the inherent challenges and limitations of a secondary or revision rhinoplasty, Dr. Ghorab is highly specialized in correcting unsuccessful or undesirable results from previous nasal surgeries. Open vs Close Rhinoplasty

 

Whether a rhinoplasty is considered open or closed depends on where the skin cuts are made.

An open rhinoplasty requires cuts to be made both on the inside and outside of the nose. External nasal incisions are designed to be small, discrete, and well camouflaged. The incisions tend to heal well and sutures removed one week after surgery. For most, the open approach leaves only a small scar at the columella (between the nostrils) that isn’t noticeable in most face-to-face interactions.

In a closed or “scarless” rhinoplasty, there are no visible external scars. All the incisions are made inside the nose and closed with stitches that dissolve on their own. The surgery is usually less involved, allowing for a quicker recovery.

Nasal Splints

 

Depending on the approach and extent of nasal surgery, nasal splints may be placed inside and/or outside the nose.

An external nasal splint or cast is placed over the bridge or dorsum of the nose. Intranasal or septal splints are thin plastic sheets that are custom-fitted to “hug” the septum inside the nose.

These splints support the nose as it heals and decrease the risk of bleeding, swelling, and fluid collection for the first week after surgery.

What to Expect with a Rhinoplasty

 

Rhinoplasty or nose job is an elective, outpatient procedure performed in our Surgery Center with licensed medical staff. In the hands of your experienced rhinoplasty surgeon in Scottsdale, nasal surgery has long-lasting results for improving the form and function of your nose.

 

Consultation

During your 1:1 consultation, Dr. Ghorab will examine your facial features and give her expert assessment for how to best achieve your aesthetic goals. If a rhinoplasty is recommended, Dr. Ghorab will review the surgical techniques, postoperative recovery, and results you can expect.

 

Anesthesia

A rhinoplasty is performed using a combination of local and twilight anesthesia. Our minimally invasive anesthesia protocol provides excellent surgical outcomes while patients are comfortably sedated and still breathing on their own. Since there is no need for a breathing tube or ventilator, the postoperative recovery is much more favorable.

 

Post-Anesthesia Recovery

After the procedure, patients are monitored in our cozy and relaxing recovery room. Once awake and doing well, patients are taken home by their designated caretaker. A custom aftercare plan is provided and follow-up scheduled.

Recovery After a Rhinoplasty

 

You’ll be able to appreciate your refined nose right away! The beautiful results will continue to improve with time. We place great emphasis on postoperative follow-up care, ensuring you are closely monitored for optimal healing.

Pain Control

Any discomfort after nasal surgery is controlled with a combination of prescription and over-the-counter pain medications.

 

Swelling, Bruising, & Bleeding

It’s normal and expected for every nose to develop some degree of postoperative swelling, bleeding, and bruising. The nose may appear larger/bigger and feel more congested, especially for the first few weeks after surgery. This improves quickly in the first few weeks to months, with more gradual resolution up to one year or longer.

 

Nasal Splint & Suture Removal

Any nasal splints and external skin sutures are removed 1 week after surgery. Only absorbable sutures are used inside the nose, so they will dissolve on their own.

Time Off

Following a rhinoplasty, we recommend around 7 to 10 days off to hunker down for R&R. Most people feel comfortable going out about 1 week after surgery when splints and sutures are removed.

 

Activity

Strenuous activities must be avoided for at least 2 weeks after surgery. Contact sports or activities, with possibility of direct impact to the nose, must be avoided for 1 month.

 

Follow-Up

Routine postoperative visits are scheduled with Dr. Ghorab, starting the day after surgery and for up to one full year.

FAQ: Rhinoplasty

Absolutely! Though there is a distinction between a “cosmetic” and “functional” rhinoplasty, surgical changes made to improve the appearance of a nose will invariably affect its function and vice versa.

A good rhinoplasty surgeon will take both factors into account when performing nasal surgery. Even still, it’s important to let your surgeon know what your concerns are and what you hope to achieve with a nose job.

The #1 reason for undergoing a functional rhinoplasty is to breathe better. Besides defining the look of the face, the nose plays an essential role in breathing. Developmental issues or anatomical defects present from birth (i.e. deviated septum), nasal trauma or injuries, medical conditions, and/or poorly executed nasal surgeries can all impair breathing through the nasal passages.

The goal of a functional rhinoplasty is to correct these issues, resulting in a higher quality of life because of easier breathing during daily activities and nighttime sleeping.

A standard open rhinoplasty requires an external skin incision or cut at the columella (skin between the nostrils when looking from below). For most people, this is the only visible cut on the outside of the nose. In some cases, additional or different external skin incisions may be used to best achieve individual aesthetic or functional goals.

Irrespective of their location, all external incisions are thoughtfully planned, well-camouflaged, and over time fade into discrete scars that are usually unnoticeable.

Based on current evidence-based research and her personal experience of performing hundreds of nasal surgeries, Dr. Ghorab does not use nasal packing after a rhinoplasty. Instead, she places custom-fitted plastic splints inside the nose for one week after surgery. Intranasal or septal splints without any nasal packing results in a significantly better recovery with far less nasal blockage and nasal/facial pain.

The purpose of intranasal or septal splints after a rhinoplasty is to support the nose as it heals and decrease the risk of bleeding and fluid collection for the first week after nasal surgery.

Also, intranasal or septal splints do not block the nasal passages, making it easier to breathe through the nose without the added pain of having the nose “stuffed” with packing material.
